Church Kids Registration Form

The Church Kids Program at St. George's On-the-Hill Anglican Church Welcome's You and we're delighted to have you aboard!

Church Kids is an exciting program for children from Kindergarten to Grade 5 (age 4 - 10) who participate in a weekly program called Godly Play which encourages the exploration of faith through fun activities, drama, storytelling, active listening and involvement. In addition the Church Kids are involved in their community and in the parish, participate in events and activities and in the annual Christmas Pageant.

All are welcome. We would love to get to know you better and would love to hear more about your interests!

It is important that for all children and families participating in programs at the parish that we have your child's information on file both in case of emergencies and to ensure that we can connect with you about all the latest information on events and activities.

  • Photo Release

    St. George's on-the-Hill has a website, social media channels (Facebook, YouTube, Instagram and Twitter) and newsletter where photos and videos from Church & Youth Group events are periodically published.

    This waiver grants St. George's on-the-Hill Anglican Church permission to use this Church Kids Participants likeness in photographs, video, and other media in any and all of its publications, websites or social media channels indicated above. In addition the right to royalties or other compensation arising or related to the use of the photographs or videos is waived.
  • Trip / Activity Permission

    Please Note that participants, parents and guardians willbe notified in advance about the dates and destinations of all trips and activities.

    This Church Kids member has my permission to participate in all field trips and activities with the Youth Group at St. George's on-the-Hill for the September 1, 2016 to August 31, 2017 Program Year, unless I specify otherwise verbally or in writing.
  • Participation Waiver

    I do hereby consent for this youth member to participate as a member in the St. George's on-the-Hill Anglican Church - Church Kids Program. I acknowledge that through participation in regular activities, trips or events there is the possibility of injury due to hazards beyond the control of the St. George’s on-the-Hill Leadership team. In consideration of St. George’s On-The-Hill Anglican Church, The Anglican Diocese of Toronto and the Anglican Diocese of Canada permitting this Church Kids member to participate in this activity. I agree to release and discharge and to indemnify and save harmless the Church and its leadership team, volunteers, representatives and chaperones from and against all claims or proceeding by whomsoever made or brought, in respect of any costs, losses, damage or injury arising by reason of my/their participation in such activities, or by reason of the provision of medical care to me/them.

    I certify that my son/daughter is in good physical condition and able to safely participate in all aspects of this program.

    I further authorize St. George’s On-The-Hill Anglican Church and its representatives to consent for and obtain such medical care to the person listed above, as it may deem necessary in the event of injury or otherwise, and agree to pay for all expenses incurred thereby.
